Updates From the Ottawa Health Team – Équipe Santé Ottawa (OHT–ESO) : Primary Care Partner Table

The Ottawa Health Team – Équipe Santé Ottawa (OHT–ESO) is made up of family doctors and nurse practitioners, and other health care partners from neighbourhoods across the city motivated to make changes to improve our healthcare system and create solutions that improve care delivery to patients. In Year 1, the focus is on serving frail older adults (55+) and adults with mental health and addiction issues.

The Primary Care Partner Table provides leadership in the primary care sector to support effective engagement and planning that align with the vision and commitments of the OHT-ESO.

Engagement opportunities include:

  • participation in co-design through action teams
  • focus groups or forums and/or
  • other initiatives that advance change.

Updates From The Table – Covid Community Response

  • The OHT-ESO has provided 7 days a week of mobile testing for vulnerable residents unable to attend assessment centres.
  • The OHT-ESO has provided wraparound supports (food, cleaning supplies, baby supplies, etc.) to COVID positive patients facing barriers to isolating.
  • The Primary Care Partner Table recruited 24 different physicians and 5 nurse practitioners to vaccinate over 1000 residents in 14 different Ottawa Community Housing buildings over 10 days in the spring. Read more here.

Community Health Team Trial

The Community Health Team Trial has launched and will be tested over a six (6) month trial period, starting mid-June and running into November 2021. The trial started with clients in the Ledbury-Heron Gate-Ridgemont neighbourhood or Community Health Area. The new model of care for the community, called Community Health Teams, was designed by health care providers and lived experience partners together to improve the care and support system for our frail older adults 55+ years of age who are losing functional independence and require enhanced support for them to age in place, in their community. The intention is to run through several cycles and scale the model across the OHT and expand across other populations.
