Subject: Updated PHO AEFI Reporting Form
Dear Colleagues,
In Ontario, Public Health Ontario (PHO) leads the provincial surveillance of adverse events following immunization (AEFIs). Health professionals are required to report AEFIs to their local public health unit.
Public Health Ontario (PHO) has recently updated the AEFI reporting form on their website. Please replace any previous versions of the form with the current July 2025 version.
Updates to the reporting form include:
- Integration of select COVID-19 Adverse Events of Special Interest (AESIs) into general AEFI categories
- Revisions to adverse event types to align with the updated national reporting form from the Public Health Agency of Canada

Completed AEFI Reporting Forms for Ottawa residents can be submitted through an online form to Ottawa Public Health or by fax to 613-580-9660.
