By the end of the educational activity, participants will be able to:
- Review and integrate newly published data and guidelines that affect how hypertension and kidney care should be delivered
- Describe and evaluate pathways for testing and management of hypertension and chronic kidney disease
- Describe ways to improve primary care referral practices and patient outcomes
