18-Month Well-Baby Visit

18-month enhanced well-baby visit (OHIP: A002, A268) provides healthcare providers an opportunity to identify growth and development concerns by using the Looksee Checklist® (formerly Nipissing District Developmental Screen®) and Rourke Baby Record. Based on your findings, refer your patients to specialized services for early intervention.
