Public Health Nurse Home Visiting

Healthy Growth and Development offers a free home visiting program for families, from pregnancy until the child goes to school. Service is offered in the language of the patient.

Patients who may benefit from our home visiting program include (but are not limited to):

  • Newcomers (less than 5 years in Canada)
  • Teenage parents
  • Families living in shelters
  • Patients dealing with addiction and/or mental health issues

After an initial assessment with the mother (questionnaire completed in person , at home), and based on needs and goals identified with the family, we offer home visiting by a Public Health Nurse and, as needed, involve our Social Worker or Family Visitor.

This is a voluntary program; the family must consent to the referral to Ottawa Public Health.

How to refer to the Healthy Babies Healthy Children home visiting program:

If you are a physician :

  • Submit the online physician referral form or
  • Call Ottawa Public Health at 613-580-6744, press 1 to indicate you wish to be served in English, and then 2 to identify yourself as a health care provider. You will be prioritized to speak with a public health nurse.
